Infrared Sauna Agreement- ***Please read the following before booking your session. Health Declaration / Contraindications I understand that Infrared Sauna use involves heat exposure which may place strain on the cardiovascular system, fluid/electrolyte balance, and other body systems. I declare if any of the following apply, I will seek medical advice that sauna use is safe for use. -Pregnancy or trying to become pregnant -History of heart disease including- unstable angina, recent heart attack , severe aortic stenosis -Uncontrolled hypertension or hypotension -Pacemaker or implantable cardioverter-defibrillator -Recent surgery or injury, especially joint injuries or wounds -Skin conditions with open lesions or infections -Fever or current infection -Hemophilia or other bleeding disorders -Sensitivity to heat (including conditions impairing sweating, e.g. neuropathy) -Severe respiratory disease (COPD, asthma if uncontrolled) -Neurological disorders, especially seizure disorders -Use of medications that affect heat regulation (diuretics, beta-blockers, antihistamines, etc.) Rules & Safe Use Acknowledgement • I will limit my session(s) to no more than 30 minutes per use. • I will stay well hydrated, drink sufficient water before and after. • I will avoid using the sauna if I feel dizzy, light-headed, overheated, ill, after alcohol or drug use. • I will exit immediately if I feel any discomfort. Release of Liability In consideration for being permitted to use the Infrared Sauna at Miscellany Greene Studio, I hereby waive, release, discharge, and covenant not to sue Miscellany Greene Studio, its owners, operators, employees, agents, successors, from all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my use of the sauna facility, including but not limited to any loss or damage, and any claim or damage on account of injury to person or property, whether caused by negligence of the Released Parties or otherwise, to the fullest extent allowed by New York State law. We will review and sign at the time of your first visit. Thank you for being safe!
